After an appointment, the QCP Coordinator uses the Patient Encounter Worksheet to enter information
into the program through the Encounter Worksheet Data Entry screen.
Many of the selections in this screen are the same as selections found in the Patient Information screen.
This is to facilitate easy access by the user.

Identification
The Identification block contains information about the visit; date, time, status, etc.

Reminders
The Reminders block lists any Reminders that are upcoming for, or have recently been sent to the patient.

Pending Results
The Pending Results block lists pending test results for the patient.

Reason for Visit
The Reason for Visit block is shown here for continuity with the PEW. The only information that can be entered into this block, in the program, is the EDC. On the printed PEW, this block is used for provider prompts based upon the Reason for Visit and for physician notes.

Health Maintenance Recommendations
This block allows the user to run an HMR Check for the patient, Enter New Test Dates or Edit HMR Data.

Health Risk Factors
The Health Risk Factors block lists all of the relevant personal and family health history items for the patient.

Comments
This block provides the user a means to enter and edit brief notes that will be seen on all future PEWs. For example; "mail returned - check address" or "check mole on back of neck".

Signature
A Signature block is provided on the PEW and is shown here for continuity only.

Outcome Screens
One of QCP functions is to track adverse visit Outcomes. Each visit requires at least one outcome to be checked. Although most outcomes will likely not be of Quality Assurance significance, adverse outcomes can be reported on for Mortality and Morbidity meetings.
