Demographics
The Demographics screen contains the patient's name, age, sex, social security number, address, phone numbers,
primary physician, insurance, etc.
Appointments and Visits
The Appointments and Visits selection provides a list of all appointments and visits for the patient, including date, time,
reason for visit, status of visit and treating physician.
Health Risk Factors
The Health Risk Factors screen lists all of the relevant personal and family health history items for the patient.
HMR Check, Last Run
This selection runs a check, only for the selected patient, on the patient's Health Risk Factors, age and sex, and generates a list of Health Maintenance
Recommendations for the patient.
Health Maintenance Recommendations
The Health Maintenance Recommendations listed for the patient can be reviewed and edited through this screen,
if the patient's needs are different from the standard clinic recommendations.
Reminders Upcoming/Sent
The Reminders Upcoming/Sent screen lists any Reminders that are upcoming for, or have recently been sent to the patient.
Pending Results
The Pending Results selection generates a list of pending test results for the patient. Follow up letters can be generated from this screen.
Miscellaneous Letters
The Miscellaneous Letters selection generates a list of Miscellaneous letters that are due to be or have recently been sent
to the patient. This includes Missed Appointment letters.
ID Numbers
Since some patients may have multiple ID Numbers, either due to marriage, changes in insurance, or work injury claims,
QCP allows lookup by all ID numbers.
Print Patient Status Report
This selection will print a one-page report of the patient's Health Maintenance Recommendations, recent visits, outcomes, etc.
Immunization Certificate Print
This selection prints a one-page report of the patient's immunization status. This is especially handy for for children returning to school!
Archived Letters
This screen lists special Reminders, Follow Up and Miscellaneous letters sent to the patient in the past.
EDC
The EDC date for a pregnant patient can be entered here. This feature can be used strictly for information, or may be set to change all of the due dates for the
patient's Health Maintenance Recommendations to a date after the EDC.
